On the playground, observed the slides containing a gap large enough (approximately 5/8 of an inch) to cause a potential strangulation hazard between the platform and slide.
Required correction: Keep all playground equipment free from entrapment and strangulation hazards at all times. Correct immediately and respond by 12/17/2025. Correct by 12/10/2025

Identified through Director J.'s statement that Family Service Advocate Maria (DOH: 2/14/22) is responsible for the collection, review and maintenance of immunization records. Reviewed Maria's file and observed it to be missing current documentation of completion of CDPHE immunization training. Reviewed PDIS electronic training records and observed it to be missing documentation of completion of CDPHE immunization training within the last 365 days.
Required correction: Obtain current documentation of completion of CDPHE immunization training for all staff responsible for the collection, review and maintenance of immunization records. Correct and respond with verification of compliance by 2/22/25. Correct by 02/22/2025
