Reviewed 14 staff files and identified one staff member to be missing the First Aid/CPR Module and with expired First Aid/CPR certificate.

Required correction: Staff not required by rule to be certified, must complete the First Aid/CPR Module. Correct immediately and submit documentation with written statement of compliance to specialist. Correct by 04/07/2026

Reviewed 8 child files and identified one child to be missing a medical statement and one child with expired medical statement.

On the Preschool Playground, exposed weed covering in the sand box, debris such as bandaid and small pieces of paper in the wood chips area, and a large bag of open garbage next to the wall, accessible to children, posing a potential hazard.

Required correction: The outdoor area must be checked and kept safe and free of hazards. CORRECTED AT THE TIME OF THE INSPECTION. Staff raked the sand box, removed the debris and the bag of garbage. No response required. Correct by 04/07/2026
