Reviewed the Director's file and the director was lacking a current immunization training.

Required correction: Within 30 days and annually, all staff responsible for the collection, review, and maintenance of the child immunizations records must complete the Colorado Department of Public Health and Environment immunization course. In the toddler classroom there was a container of diaper cream labeled "keep out of reach of children" located in an unlocked cabinet accessible to children.

In the toddler classroom there were several handmade sensory bottles. One of the sensory bottles was easy to open and contained small bead. The one sensory bottle was sitting on a low shelf accessible to children under the age of 3. This was posing a possible choking hazard.

Required correction: Toys, toy parts, furnishings, equipment, and any materials accessible to children under than three (3) years of age must not be a choke hazard or able to be inhaled. Any area of the facility accessible to children less than three (3) years of age must be free of any choke or inhalation hazards. ***Corrected at the time of the visit the director removed the sensory bottle from the low shelf and made it inaccessible to children on 3/5/2026. Respond with a plan of ongoing compliance by 4/5/2026. Correct by 03/05/2026

Outside on the brown fence there were two nails protruding from the fance posing a possible safety hazard. Outside on the infant/toddler playground there were two cracked and broken white chair platforms that were posing a possible safety hazard. Outside on the infant/todder playground there was a red dust pan was cracked and sitting on the ground posing a possible safety hazard. Outside on the infant/toddler playground there was a wooden sun shade area that serveral areas of green peeling paint. The peeling paint is posing a possible safety hazard. Outside on the preschool playground there was a pink shed and there was peeling paint on the door of the shed. Outside on the preschool playground there was an area of the pour and play that was speadind apart and posing a possible tripping hazard. Outside on the walkway from the preschool classroom to the playground there was a large area of exposed weed barrier fabric that was posing a possible tripping hazard.

In the preschool classroom there was a bottle of disinfectant spray located on a cubbie that was accessible to children. Disinfectant wipes were located in a lower cubbie near the window accessible to children. In a closet that is attached to the preschool classroom there were two large bottles of hand sanitizer located on the floor accessible to children.

Required correction: All items labeled "keep out of reach of children" including disinfectant spray, disinfectant wipes, and hand sanitizer must be inaccessible to children. ***Corrected at the time of the visit the staff members placed all items up high

In the preschool classroom there were 2 pairs of adult scissors located on a low cubbie accessible to children.

In the preschool classroom there were three cords longer than 18 inches long in the Ipad station that were accessible to children. The long cords were posing a possible safety hazard.
