Reviewed 11 staff files and identified E.C., J.W., S.F., and M.W. with expired annual self-reported history and M.R. to be missing a health statement. Ezgi, J., Shepreasa and Mary signed the annual self-reported history at the time of the inspection.
Required correction: All staff must submit a medical statement then a self-reported health history must be submitted annually. Submit a written statement of compliance to specialist. Correct by 07/10/2025

Required correction: Medication that is out of date (expired) and parents do not pick up the center must dispose of the medications as required by CDPHE. Correct immediately.
