Reviewed drill logs and observed that they were missing documentation of a fire drill completed in November and December 2025 and a tornado drill completed in September 2025.

Required correction: Immediately, drills must be complete within the required times and a record must be maintained by the facility including date and time of drill, number of adults and children participating, and the amount of time taken to evacuate. Submit a written statement of compliance to the Licensing Specialist. Correct by 03/12/2026

On the playground, observed a small trampoline accessible to children to play on.

Required correction: Immediately, trampolines and inflatable bouncers are prohibited. Corrected at the time of the visit. The director moved the trampoline off of the playground making it inaccessible. No response is required. Correct by 05/19/2025

Required correction: Immediately, items labeled "keep out of reach of children" must be inaccessible to children. Corrected at the time of the visit. The teacher moved the wipes to a high cabinet making them inaccessible to children. No response is required. Correct by 05/19/2025
