Required correction: Ensure all items labeled "keep out of reach of children" are inaccessible to all children at all times. ***Corrected at the time of visit. Director immediately placed the staff purse in an out of reach cabinet and replaced the broken lock on the laundry cabinet door, making all items inaccessible to children. NO RESPONSE NEEDED. Specialist measured less than one inch of wood chips in several areas around the climbing equipment that is over (18) inches high on the preschool playground. Specialist also observed pea gravel had blown into the wood chips, rendering the wood chips ineffective as resilient material.
Required correction: Ensure all climbing equipment (18) inches or higher has at least (6) inches of resilient surfacing throughout the use zone surrounding the equipment. Program understands the climbing equipment must be made inaccessible to children until resilient surfacing measures at least (6) inches. Correct immediately and document correction in written response to Specialist by 1/29/2026. Correct by 01/22/2026
