A wood chunk on the ground accessible to children in the backyard with a rusty metal nail protruding from it.

Required correction: All outdoor areas must be kept safe and free from hazardous materials that could cause harm to children. Correct by 6/2/26. The Provider removed the wood chunk with the nail and threw it in the trash in the garage while the Licensing Specialist was present. Corrected at the time of inspection. No response necessary. Correct by 06/02/2026
