(DOH 2.18.2023) all background checks expired in 2024. Observed W.M. (DOH 7.26.2012) all background checks expired in 2024.
Required correction: Obtain the required FBI background checks for all staff members as required. Correct immediately by 1.20.2025, and submit a written statement of compliance and documentation of the backgrounds to the Specialist by 1.27.2025. Correct by 01/20/2025
