At the time of the visit, in the Cubs classroom, all musical instruments were inaccessible to children. In the Duckie's classroom, manipulative tools, puzzles and art supplies and music were inaccessible to children. In the Bees classroom, the only items accessible to children were books and art.

Required correction: Immediately ( 9/10/2025), play materials must be readily accessible so that children can arrange, select and remove materials independently. **Corrected at the time of the visit when all teachers in the classrooms, uncovered the learning materials. Submit documentation and a future statement of compliance by 9/17/2025. Correct by 09/10/2025

Reviewed 14 staff files. Observed M.T., hired 10/2019, C.S., hired 6/26/2022, M.F., hired 4/15/2022, and M.W., hired 8/5/2022, lacking documentation of a current standard precautions training.

Required correction: Review all staff files. All staff must complete a Department-aproved standard precautions training prior to working with children and must renew annually. Submit documentation and a written statement of compliance by 10/1/2024. Correct by 09/24/2024

In the Bees classroom, wipes were on the counter, accessible to children. In the Lions classroom, wipes were on a low cubby with art supplies, accessible to children.

In the Duckies classroom, a child's medication was lacking the first and last name.

Required correction: Immediately, medication must be labeled with the child's first and last name. Corrected at the time of visit, the teacher labeled the medication with the child's first and last name. Submit a written statement of future compliance by 10/1/2024. Correct by 09/24/2024

In the Duckies classroom, the teacher stated they had 15 children, however 14 children were in attendance. In the Junior Kindergarten classroom, 18 children were in attendance, however 16 were signed in.

Required correction: Immediately, the staff must know how many children they have at all times and the attendance must be updated to reflect the amount of children are in the classroom. Corrected at the time of visit, the teacher corrected the attendance log to state 18 children were in attendance. Submit a written statement of future compliance by 10/1/2024. Correct by 09/24/2024
