Required correction: Hazardous items and materials must be inaccessible to children. Corrected at time of visit- Provider moved items to a high shelf making items inaccessible on 4/30/26. No response required. Correct by 04/30/2026

Notify the Licensing Specialist in writing once the resident has completed the fingerprinting process. Correct by 06/09/2025
