The following items labeled 'keep out of reach of children' were accessible to children: In Room 16, a bottle of white out was in an unlocked drawer. In Room 17, a bottle of Purell Sanitizer Spray was in an unlocked cabinet under the sink.
Required correction: Immediately, items labeled keep out of reach of children' must be inaccessible to children at all times. Corrected at time of visit, the drawer in Room 16 was locked by the classroom teacher and the bottle of sanitizer spray in Room 17 was placed in a high cabinet by the classroom teacher making the items inaccessible. No Response Needed. Correct by 04/29/2026

In Room 16, a box cutter and staple remover were in an unlocked drawer and accessible to children. In Room 21, three pairs of adult scissors were in an unlocked drawer and accessible to children.
Required correction: Immediately, sharp tools must be inaccessible to children at all times. Corrected at time of visit, the drawers in both classrooms were locked making the items inaccessible. No Response Needed. Correct by 04/29/2026

The following electrical cords were identified and presented a strangulation hazard to children: In Room 3, a lamp cord and other electrical cords were accessible to children. In Room 17, a long electrical cord was dangling from the ceiling. In Room 20, an electrical extension cord was in an unlocked drawer, and the tile floor around the water table was wet creating a slipping hazard.

The spinning seat and the swings on the playground near the building to have as little as one inch of resilient surfacing. Observed the weed matting-used to prevent weeds from growing- to be exposed due to the lack of woodchips.
Required correction: There must be at least six inches of resilient surfacing surrounding all fall zones on equipment that measures over 18 inches in height. Correct immediately by 6.25.2025. and submit a written statement of compliance to the Specialist by 7.2.2025. Correct by 06/25/2025
