At the time of visit, in Toddler 2, a toddler was drinking a bottle while laying on their mat for nap time.

At the time of visit, in Toddler 3, there were 8 children in care and only 7 children on the attendance roster. Throughout the facility, staff were not tracking transitions.

At the time of visit, in the Preschool 3 classroom, the attendance roster stated there were 10 children in care, but there were only 9 children in the room.

Required correction: Immediately, the time a child arrives and leaves the center each day must be recorded. Corrected at time of visit, the classroom teacher signed out a child who had left for the day, correcting the attendance roster. Submit a written statement of future compliance by 12/2/2023. Correct by 11/02/2023

At the time of visit, in the Infant 1 classroom, a mattress had a loose sheet; no infant was sleeping in the crib.
