The staff working with a child with a health care plan must be informed, trained, and delegated responsibility for carrying out the health care plan by the Department-approved child care

Reviewed 8 staff files and observed 1 staff (M. F.) lacking documentation of a medical exam signed by a medical provider dated with in 6 months of the hire date.

Required correction: Provide a written statement of compliance ensuring that all staff members submit to the center a medical statement, signed and dated by a physician or other health care provider that is dated no more than six months prior to employment or within thirty calendar days after the first date of employment. Subsequent self-reported health histories must be submitted annually. Correct by 02/04/2024
