A pack 'n play had a loose sheet on the mattress. No child was sleeping in the pack 'n play at the time of visit.
Required correction: Immediately, mattresses for cribs must have a properly fitted sheet. Corrected at the time of visit, the provider removed the sheet. Submit a written statement of future compliance by 4/28/2024. Correct by 03/28/2024
