SM8 & SM10 working together in the Infant 1 room at time of inspection. Reviewed 10 staff files and observed no documentation of CPR certification for SM8 or SM10.
Required correction: There must be at least 1 staff member on duty in each infant room at all times who hold CPR Certification. CORRECTED AT TIME OF INSPECTION - SM3 who has CPR Certification was moved into the Infant 1 room. Correct by 05/01/2026

Bottom of gate door on toddler playground to be broken and pulling away from framing exposing sharp metal edges and leaving a gap of approximately 4-6 inches accessible to children and posing a potential hazard.
Required correction: Immediately make all hazardous areas and items inaccessible to children. Submit documentation of repairs with written response to specialist. Correct by 05/01/2026

Required correction: Immediately make all hazardous items inaccessible to children. CORRECTED AT TIME OF INSPECTION - staff member moved Tylenol and Magic Eraser sponges to a high cabinet making it inaccessible to children. No response required. Correct by 05/01/2026
