A child file that was reviewed during a previous inspection still did not have documentation of immunization status during today's visit on 4/1/2025. On the provider's response form dated 6/15/2024, the provider stated that all parents had returned emergency medical authorization forms. During today's inspection on 4/1/2025, three children's files that were reviewed on the last inspection, still had expired emergency medical authorization forms.
