Attached to the crib bars on the inside of the crib, observed a protective covering made of fabric intended to protect an infant's arm from becoming stuck between the bars. The protective covering was bunched up close to the mattress, posing a potential suffocation hazard. No child was observed in the crib at the time of visit.
Required correction: Ensure all soft bedding or materials that could pose a suffocation hazard are not permitted in cribs or other sleep/rest equipment for infants. **Corrected at the time of visit. The provider removed the protective coverings.** No response required. Correct by 11/20/2024

Fingerprints were originally done under the wrong license number due to a miscommunication: CBI dated 6/4/22. A portability form was completed within 180 days and not sent into the BIU.
Required correction: All individuals requiring a background check must review 'Navigating the Background Check', the Investigation Unit's (BIU) Webpage powerpoint on the CDEC website. Submit verification (via email) that Provider Nadia has reviewed the powerpoint within 24 hours. Correct immediately and submit documentation of CBI and FBI clearance letters for P. with written response to Specialist by 11/2/23. Correct by 10/26/2023
