Four small wool balls and one plastic toy duck that fit in a choke tube on a lower shelf accessible to children in the Crestone Peak toddler classroom. Observed 2 small rocks and 2 small plastic birds on a lower shelf, accessible to children, that fit in a choke tube in the Sunlight Peak toddler classroom.

Required correction: Toys, toy parts, furnishings, equipment, and any materials accessible to children under than three years of age must not be a choke hazard or able to be inhaled. Staff removed all of the items from the classroom and relocated them to the front desk area that is inaccessible to children. Corrected at the time of inspection. No response necessary. Correct by 09/08/2025

Observed on the preschool playground the bottom area of 2 green slides to have less than 2 inches of resilient surfacing. Observed on the toddler playground the bottom area of 2 red slides to have less than 3 inches of resilient surfacing.

Required correction: All climbing equipment eighteen (18) inches or higher must have resilient surfacing of at least six (6) inches in the use zone surrounding the equipment. Correct by 10/24/24. Document compliance to Specialist by 11/24/24. Correct by 10/24/2024
