Identified through staff statements and review of center policies, at the time of the incident the procedure for informing parents of accidents and injuries had not been updated to reflect current practices.
Required correction: Immediately, changes to center policies and procedures must be developed, implemented, and followed at the time of the change. Submit documentation of an updated accident and injury policy and a written statement of compliance to the licensing specialist. Correct by 04/14/2026

In the UPK classroom, a bottle of Sanitizer labeled 'keep out of reach of children' was on a shelf near an activity table and was accessible to children.
Required correction: Immediately, all items labeled 'keep out of reach of children' must be inaccessible to children at all times. Corrected at time of visit, the classroom teacher moved the sanitizer to a high hook making them inaccessible. No response needed. Correct by 04/14/2026
