Reveiwed provider file and observed an expired Colorado Department of Public Health and Environment (CDPHE) immunization course. Training was last completed on 1/26/2025.

Required correction: All individuals responsible for the collection, review, and maintenance of the child immunization records must complete the Colorado Department of Public Health and Environment (CDPHE) immunization course. This training must be renewed annually. Respond with statement of compliance. Correct by 08/07/2026

Reviewed 8 child files and observed 2 with an expired written authorization for emergency medical care. Emergency medical care was last signed by a parent or guardian on 12/15/2024 and 6/16/2025.

Reviewed 8 child files and observed that 3 files lacked a current statement of childs health signed and dated by a health care provider. The 3 child files are enrolled for over 30 days, and the health statements on file are expired by more than 30 days.

Required correction: The statement of the child’s current health status must be signed and dated by a health care provider who has seen the child within the last twelve (12) months, or within the last six (6) months for children under two and one-half (2-1/2 ) years of age.
