Identified through staff statements and review of video documentation that SM1 confined CH1 to a playpen while on the playground. At the time of the inspection SM2 stated that CH1 "could very well have been confined in the playpen for longer than 15 minutes"

Identified through staff statements and review of video documentation that SM1 harshly removed CH1 from the playpen and roughly placed CH1 on the ground, SM1 also hit CH1's shoe out of their hand when CH1 approached SM1 for help.
Required correction: Immediately (6/2/26) ensure all children are not subjected to physical or emotional harm, humiliation, or threats. Provide a written plan of action along with a written statement of compliance within the overall response. Correct by 06/02/2026

The following items labeled "keep out of reach of children" accessible to children: Hand lotion and a bottle of disinfectant by the sink in the toddler classroom, a spay mop next to the toilet in the toddler room, sunscreen, cough drops and an essential oil spray in a basket next to the outside door in the toddler room.
Required correction: Immediately (12/20/23) ensure all items labeled Keep out of reach are in accessible to children at all times. Staff moved all items listed above to an in accessible location at the time of the inspection. NO RESPONSE REQUIRED.
