Observed two bottles with unsecured lids stored on a low shelf in the Sugar Beets classroom. The bottles contained small beads that present a choking hazard and were accessible to children under three years of age.

Required correction: Any materials accessible to children under the age of 3 must not be choke hazards. ***Corrected at the time of inspection. Staff removed the 2 bottles containing the small beads from the classroom. No response required. Correct by 02/17/2026

Observed a climbing structure in the 1 year old classroom and another identical structure in the 2-3.5 year old classroom both measuring over 18" in height (approximately 20") and lacking resilient material.

Required correction: All climbing equipment 18" or higher must have resilient surfacing of at least 6 inches in the use zone surrounding the equipment. Equipment must be made inaccessible until resilient material is installed. Correct by 03/12/2025

Observed a climbing structure in the 1 year old classroom and another identical structure in the 2-3.5 year old classroom both lacking a 3 foot use zone surrounding the equipment. Both structures included a slide and both lacked a 6 foot use zone extending out from the base of the slide.

Required correction: Toddler climbing equipment must have a 3 foot use zone surrounding the equipment. Toddler slides require a 6 foot use zone extending out from the base of the slide. Equipment must be made inaccessible until use zone is acquired. Correct by 03/12/2025

Observed the 2-3.5 year old classroom teachers to not complete a name-to-face after their transition into the classroom from the playground. Observed the attendance tracking system to show 10 children when there were only 9 children present in the classroom.

2.230.C.1.a · Correction due April 3, 2024

Climber with slide on the playground was observed with no resilient surfacing under the equipment.

Required correction: The director immediately, 4/3/2024, removed play structure from the playground during the time of inspection. No response required. Correct by 04/03/2024
