Required correction: Immediately, items labeled 'keep out of reach of children' must be inaccessible to children at all times. Corrected at time of visit, the Center Director put the soap bottles in a high cabinet making the items inaccessible. No response needed. Correct by 05/14/2026

In the Wild Things classroom, art projects on the wall were made of cotton balls which are a choke hazard for children under 3 years old.

Required correction: Immediately, all materials accessible to children less than 3 years old must not be a choke hazard at any time. Corrected at time of visit, the assistant director removed the art projects from the classroom. No response needed. Correct by 05/14/2026
