Reviewed 12 staff files and observed L. to be lacking documentation of out of state clearance letter. Facility obtained documentation from the state, but did not submit to Colorado BIU.

Required correction: Background check requests must be submitted and successfully completed for each state an individual has resided in, in the past 5 years, prior to an individual caring for children or allowing unsupervised access to children. Correct by 08/08/2024

Required correction: All employees must be trained on child abuse prevention and this training must be renewed annually. Correct by 08/08/2024

Reviewed 12 staff files and observed B. to be lacking 8 hours of infant orientation and is aide qualified, working in the toddler classroom.

Required correction: Staff aides must work directly under the supervision of the Director or an ECT and must have completed 8 hours of orientation in the toddler program. Correct by 08/08/2024
