At the time of visit, observed on the outdoor play area deck raised tarp and rugs creating a tripping hazard.

Required correction: Immediately. All outdoor play areas must be kept safe and free from materials that could cause harm to children. Corrected at time of visit. Provider moved the tarp to cover the deck slats and flattened out the rug. Submit written statement of future compliance by 5/23/2026. Correct by 04/23/2026
