In Room 2 and Room 4, charging cords were accessible to children creating a safety hazard. On the playground, the tunnel structure has gaps at the seams creating a safety hazard.

Required correction: Immediately, all indoor and outdoor equipment and furnishings must be sturdy, safe, and free of hazards at all times. Correct by 10/13/2025

In Room 2, a box of loose ziploc bags near the sink was accessible to children, and a loose garbage bag was in a file organizer and accessible to children.

Required correction: Immediately, all loose plastic bags must be inaccessible to children at all times. Corrected at time of visit, the center director moved the plastic bags to high shelves making them inaccessible to children. No response needed. Correct by 10/13/2025

In Room 2, there is chipped and peeling paint on the wall in the cozy corner. On the playground, there is paint peeling off the tunnel structure creating a safety hazard.

The "Emergency" Bag was hanging on a fence post easily accessible to children.

In Room 2 soap on top of the paper towell dispenser, hand sanitizer, and teacher scissors on a lower shelf, whipped cream in the fridge, and wipes in the bathroom, labeled "Keep Out of Reach of Children" to be accessible to children.
