Outside in the backyard there was a broken piece of a play kitchen with two screws sticking out the sides of the play kitchen posing a safety hazard.

Required correction: All outdoor play ares shell be surveyed and must be kept sage and free of hazardous materials including the two screws located on the side of the play kitchen. ***Correct at the time of the visit the provider removed the screws from the play kitchen on 12/16/2024. No response required.*** Correct by 12/16/2024

In the family kitchen under the sink there were cleaning supplies that were labeled keep out of reach of children in the unlocked cabinet. Also in an unlocked drawer in the kitchen there were several large kitchen knives accessible to children.

Required correction: Ensure that all items labeled keep out of reach as well as any hard items must be kept inaccessibe to children. ***Corrected the provider removed the knives and placed them up high so they were inaccessible to children.*** Correct immediately and respond with verification of compliance by 2/24/2024.
