Identified that both Infant 2 classroom staff had completed infant safe sleep training in 9/2025, and a child in their care was found in an unsafe sleep environment.
Required correction: Ensure that all safe sleep practices are followed, for all infants, at all times. **Corrected at time of visit - CH1 was returned to a safe sleep environment on 10/30/25.** Respond with a plan for ongoing compliance by 11/30/25. Correct by 10/30/2025

In the Infant 1 classroom, observed two infants under 12 months old (one was asleep) wearing sleep sacks that are designed to swaddle the infant. The infants were not observed to have their arms restricted. In the Infant 2 classroom, observed one sleeping infant under 12 months old wearing a "star" sleep sack that had enclosed arms and appeared to restrict arm movement.

During walk-through, the following hazardous items were observed (including but not limited to): - In Toddler 2 classroom: open paint cans, metal roller handles and rollers, an open worker's backpack containing painting tools, large cardboard box filled with trash/debris, tubes of caulk; - In 1st and 2nd floor hallways: large open black plastic trash bags containing trash/debris, metal folding ladders, paint cans, caulk gun; - In front office/entry areas: paint buckets, large open black plastic trash bags containing trash/debris; and, - In Toddler 2 classroom: stacked cardboard boxes, 1 razor blade in unsecured drawer.
Required correction: All hazardous items and items labeled "Keep out of reach of children" must be inaccessible to children at all times. Correct and respond with statement of compliance by 12/8/24. Correct by 12/08/2024
