Reviewed Provider's files and observed the program lacking documentation of a fire drill, a tornado drill and a lock down drill being conducted during the month of July 2026. The previous lock down drill recorded was 4/7/2026. Provider stated the drills were conducted on 7/6/2026 and not recorded on the drill log.

Required correction: Ensure all required drills are completed within the required time frames and documentation of each drill is maintained for at least (12) months, to include the above listed information. Correct immediately and document correction in written response to Specialist. Correct by 08/04/2026
