At the time of visit, observed in the girls restroom baby oil and baking soda accessible to children in an unlocked cabinet with a broken lock. Also observed diaper wipes labeled keep out of reach in the boys restroom on the floor of a stall.

Required correction: Immediately. Items labeled keep out of reach of children must be inaccessible to children. Submit documentation and written statement of compliance by 5/2/2026. Correct by 04/02/2026
