In infant classroom three and infant classroom infants classroom five observed loose fitting sheets in most of the cribs posing a potential safety hazard.

Required correction: Cribs must have properly fitted sheets. Corrected at the time of the inspection. The Director removed the sheet and or tightened the sheets. Corrected at the time of the inspection. No response required. Correct by 03/09/2026

Playdough labeled for children over the age of two and three in classrooms Toddler Four, Toddler Three, Twos four, Toddler twos. Observed the playdough accessible to children under the age of two and three. Observed a ball in classroom Infant classroom five labeled for children over the age of three, posing a potential safety hazard.

Required correction: A variety of play equipment and materials appropriate for children's age, size, developmental needs, and activities must be provided for both indoor and outdoor structured and free play. Corrected at the time of the inspection. The Director removed the playdough and the ball from the classrooms. No response required. Correct by 03/09/2026

Reviewed the fire inspection and observed the inspection report to be unapproved, posing a potential safety hazard.

Required correction: Obtain an approving fire inspection report. Correct immediately by 4.14.2025, and submit a written statement of compliance to the Specialist by 5.14.2025. Correct by 04/14/2025

A staff members water bottle to be made of glass in the preschool 2 classroom, and a glass mason jar in the preschool 3 classroom, both accessible to children under the age of five and posing a potential safety hazard.

Required correction: Toys, toy parts, furnishings, equipment, and materials made of brittle, easily breakable plastic or glass are not permitted for children less than five. Corrected at the time of the inspection. The glass was removed from both classrooms. No response required. Correct by 04/14/2025

Reviewed 29 staff records and observed two staff member's T.G. (DOH 3.5.2024), R. S (DOH3.11.2024) and T.Q. (DOH 5.8.2023) to be missing documentation of the required out of state background clearance letter. Identified that both staff members lived out of the state of Colorado in the past five years.

Reviewed 29 staff records and observed one staff members M. A. (DOH 9.2018) to have completed a CPR and First Aid course through an unapproved vendor and the staff member to be missing the required CPR and First Aid module.

Required correction: All employees caring for children, not required by rule to be certified in First Aid and CPR, must complete the Department-approved Introduction to First Aid and CPR module. Correct by 5.14.2025, and submit a written statement of compliance to the Specialist. Correct by 05/14/2025
