In the classroom area there was an uncovered outlet near a white shelf.

Required correction: In rooms accessible to children, all electrical outlets and power strips must have protective covers, or safety outlets must be installed; all exposed light bulbs accessible to children in areas where children can play must have protective covers. Electrical cords must be in good condition and must not pose strangulation, falling, or tripping hazards. ***Corrected at the time of the visit the provider placed an outlet cover on the outlet on 6/1/2026. In the kitchen in an unlocked cabinet accessible to children there was a large roll of plastic trash bags posing a possible suffocation hazard. In the living room there was a printer cord longer than 18 inches long accessible to children that was posing a possible suffocation hazard. On a lower shelf accessible to children there was a toy car that had 3AAA batteries exposed posing a possible safety hazard.

Required correction: All hazards items including the items listed above must be inaccessible to children. ***Corrected at the time of the visit the provider removed items placing the items inaccessible to children on 6/24/2025.***Respond with verification on compliance by 7/24/2025. Outside in the backyard there was brick tiles that were uneven had sharp edges posing a possible safety hazard. Outside near the evacuation gate there were large stepping stones with sharp edges that were piled high posing a safety hazard. There were nails sticking out of two fence posts posing a safety hazard.

Required correction: All outdoor play areas shall frequently be surveyed and must be kept safe and free from hazardous materials or debris that could cause harm to children. Correct and respond with verification of compliance by 7/24/2025. Correct by 07/24/2025
