Required correction: Within 30 days of employment all staff responsible for the maintenance of child immunizations must have an immunizations training that is updated annually. Correct immediately and provide written statement of compliance. Correct by 02/04/2026

Reviewed 13 staff files and found AJ to be missing documentation of the completed TRAILS background check. Proof of submission was in their file.

Required correction: All individuals requiring a background check must review Navigating the Background Check Investigation Unit's (BIU) Webpage powerpoint. Submit verification that AJ has reviewed the powerpoint within 24 hours. Submit documentation of a TRAILS Clearance Letter for staff AJ with written response to the Specialist. Correct by 04/24/2024
