At the time of the visit, observed Ms. J., Ms. T. and Ms. S. to be the assigned staff member to the Caterpillars classroom. Ms. A., Ms. J. and Ms. B. to be the assigned staff members to the Dragonflies classroom. Ms. K., Ms. L. and P. to be the assigned staff members to the Love Bugs classrom. Ms. B. and Ms. L. to be the assigned staff members to the Fireflies classroom

At the time of the visit, observed 8 children in the Dragonflies group of children in the playground, the staff member reported that they had 10 children. The staff members with the Fireflies group of children and the Pandas group of children weren't sure about the number of children they are caring for in the playground. Also observed 10 children in the Tigers group of children in the playground, the staff member reported that they had 11 children.

Required correction: Immediately (7/17/2025) staff must verify attendance before and after every group transition from one physical space to another. At the time of the visit observed the following safety hazards accessible to children in the following areas: - in the Baseball field, observed a wooden slat with several nails exposed and can pose a safety hazard for children. - Chicken wires with sharp edges in the playground. - A loose cord that can pose a safety hazard for children, accessible to children in unlocked drawer in the Dance room.

Required correction: Immediately (7/17/2025) the playground area must be in good repair, of solid and safe construction, free of rough edges, protruding bolts. Corrected at the time of the visit, a staff member fixed the nails and made the sharp edges of the wires inaccessible to children and removed the loose wire.
