The gate to the kitchen area was observed to be open in room 112 and room 103 making a bottle of dishsoap and a spray bottle of disinfectant cleaner labeled "keep out of reach of chidren" accessible to children.

Required correction: Immediately (5/8/2026) ensure all items labeled "Keep out of reach of children" are in accessible to children at all times. Teacher corrected at the time of visit by closing and locking the gate in both room 112 and room 103. No further response is required. Correct by 05/08/2026

Reviewed two medications and observed one to have an expired written authorization order from the prescribing practitioner.

Required correction: Immediately (5/8/26) ensure that all children who require ongoing medications have a current authorization and consent form signed by a healthcare provider. Ongoing medications are required to be reauthorized on an al least annual basis. Correct by 05/08/2026

Required correction: Immediately (5/21/25) ensure loose plastic bags must be stored in areas inaccessible to children. Teacher corrected at time of visit by placing bags on top shelf of storage out of reach of children. Correct by 05/21/2025
