In classroom 116 licensing specialist reviewed the attendance sheet, and one child was lacking the time of arrival to the program.

Required correction: The time a child arrives and leaves the center each day must be recorded. Staff members must complete written attendance verification periodically throughout the day, including during transitions. ***Corrected at the time of the visit the staff member wrote in the time the child arrived. Licensing Specialist observed 19 children in classroom 118 and 1 staff member alone at nap time. There were 13 children awake and more than half the children were not sleeping.

Required correction: At nap time, the child to staff ratio may be doubled for children 2 1/2 and older in preschool classrooms when the following conditions have been meet: At least half of the children are sleeping. Correct immediately and respond with verification of compliance by 11/19/2024. Correct by 11/12/2024

Reviewed 5 staff files and Ms. C. was lacking verification that she had completed the affidavit as well as an application in PDIS in order to be a qualified Early Childhood Teacher.

Required correction: Ensure that Ms. C. must complete the affidavit as well as ensure there is a pending application in PDIS and ensure all docuntation is on file. Review and ensure that the facility maintain verification of uqalifications and trainings. Correct and respond with verification of compliance by 1/7/2024. Correct by 01/07/2024

In classroom 116 it was observerd that Ms. C. was left alone with 15 children ages 4 to 5 years old while Ms. B. went to use the restroom. Classroom 116 was over by 3 children ages 4 to 5 years old.

Required correction: Ensure that staff to child ratio is maintained at all times.***Corrected at the time of the visit Ms. B. came back into the classroom.*** Respond immediately with verification of compliance and an ongoing plan of action by 1/7/2024. Correct by 12/08/2023

Outside on the playground there were several areas on both play structures that had exposed metal posing a safety hazard.
