On the ECE Playground, observed the right part of the left swing (facing the street) containing an "S" hook at the top connecting the swing with an "S" hook that has a gap larger than a dime, causing a potential safety hazard.

Required correction: Ensure all swings that have "S" hooks are secure at all times. Correct immediately and respond by 9/1/2025. Correct by 08/25/2025

Submit verification that SM1 and SM2 have reviewed the powerpoint within 24 hours. Submit documentation of TRAILS clearance letter for staff members 1 and 2 with written response to the Specialist by 9/1/2025. Correct by 08/25/2025
