A tricycle on the playground missing its rubber handle, and the bottom of the chain link fence's gate on the west side of the playground broken and pulling away from the frame creating an entrapment hazard.

Reviewed 4 staff files and observed no documentation of completed pre-service training for staff member M. This is a direct violation of stipulation K on the settlement agreement which states, "Staff members must complete any trainings required for their assigned position prior to being placed in the classroom. This may include, but is not limited to: training and orientation in facility policies and procedures, standard precautions, CPR and first aid, safe sleep, shaken baby/abusive head trauma prevention, orientation for a toddler program staff aide and medication administration. Verification of completed training must be available for review at all times."

Reviewed 4 staff files and observed no documentation of CPR certification or First Aid and CPR module for staff member M. One tricycle on the preschool playground missing a rubber handle cover and a plastic bucket with cracks exposing sharp plastic edges posing a potential hazard.

Required correction: Immediately make all hazardous items inaccessible to children. CORRECTED AT TIME OF INSPECTION - staff member moved tricycles to a room in the building and threw away the plastic bucket making all hazardous items inaccessible to children. No response required. Correct by 02/26/2024

At least two children from classroom 112 playing on the ECE playground equipment measuring approximately 30 inches in height with frozen resilient material (woodchips) under the equipment.

Required correction: Immediately make all hazardous areas inaccessible to children. Submit plan of action with written response to specialist. CORRECTED AT TIME INSPECTION - staff members had all children cease playing on the equipment and moved to other areas of the playground. Correct by 01/05/2024
