At the time of visit the Seahorse preschool room, and the toddler Starfish room were over ratio. The Seahorse room had 14 children with 1 staff member, and the Starfish room had 8 toddlers with 1 staff member. This is a direct violation of stipulation (B) of the probationary license. Stipulation (B) states "Licensee shall comply with required staff-to-child ratios at all times. Ratios and room capacities must be posted in each classroom."

At the time of visit, the facility did not have a menu posted in a place visible to parents. There was a change to the fruit on the menu today.

Confirmed through infant staff member statement, staff member is unable to communicate information to parents. This is direct violation of stipulation (E) of the probationary license. Stipulation (E) states, "All staff shall demonstrate knowledge of, interest in, and concern for children proper care and well-being."

At the time of the visit there was a 20-month-old child in the infant room, licensed for children ages 6 weeks to 18 months old.

Corrected at the time of the visit, the director removed the 20-month-old child from the infant room and placed them in the toddler room. At the time of the visit the Seahorse preschool room had 11 children signed in and there was 12 children present. In the infant Guppies room, they had 3 infants signed in and 4 infants were present. This is a direct violation of stipulation (G) of the probationary license. Stipulation (G) states, "Licensee will use a name-to-face attendance verification tool (paper or electronic form) and complete attendance verification for all transitions and at frequent intervals throughout the day. Licensee shall provide training for all staff on how to use the attendance verification tool and must maintain verification of the training in each employee's staff file."
Required correction: Correct immediately, the time a child arrives and leaves the center each day must be recorded. Staff members must complete written attendance verification periodically throughout the day, including during transitions. At the time of the visit, reviewed 4 staff files and found that there was not a staff member who completed medication administration training on site.
