Reviewed the facility's emergency logs. Observed missing fire drills for January and February 2026 and missing lockdown drill for the 4th quarter of 2025.
Required correction: Fire drills must be held monthly and lockdown drills must be held at least quarterly. Correct immediately and respond with verification of compliance by 3/18/2026. Correct by 03/11/2026
In the cleaning storage room, observed door to be open and loose plastic bags accessible to children and posing a potential safety hazard.

In the cleaning storage room, observed door to be open and Clorox, Lysol, Floor Science, Dawn dish soap all labeled "keep out of reach of children" accessible to children and posing a potential safety hazard. In addition, observed a mouse trap under the storage cabinets on the back wall, accessible to children and posing a potential safety hazard.
Required correction: Keep all items labeled "keep out of reach of children" inaccessible to children at all times. **Corrected at time of visit, staff member closed the door and locked it on 3/11/2026. Staff member also removed the mouse trap on 3/11/2026.** No response is required. Correct by 03/11/2026

In an unsecured storage cabinet along the back wall, observed an electrical power cord with a cord longer than 18 inches. Observed a light table cord and sound machine cord along the front wall with cords longer than 18 inches. These cords were accessible to children and posing a potential safety hazard.
Required correction: Keep all indoor and outdoor equipment, materials, and furnishings sturdy, safe, and free of hazards at all times. **Corrected at time of visit, staff member removed the cords and locked the cabinet on 3/11/2026.** No response is required. Correct by 03/11/2026
