1 blue strider bike on the preschool playground with worn/broken handlebars posing potential safety hazard, and 2 rugs in Classroom 154 with worn corners that are rolling up in 3 places, posing potential tripping hazard. All areas accessible to children.

Required correction: Immediately ensure indoor and outdoor equipment, materials and furnishings are sturdy, safe and free of hazards. Correct by 03/02/2026

Identified through staff statement that attendance tracking sheet was accidentially left in the cafeteria. Observed children had transitioned from the cafeteria to the classroom and written verification of attendance was not completed when the class transitioned from lunch back to the preschool room.

Required correction: Immediately ensure written attendance verification is taken periodically throughout the day, including during each transition. Corrected at the time of visit. Staff corrected attendance tracking to show all children were present during the transition. Document continued plan of compliance in written response. Correct by 04/10/2024
