On the playground, observed three classrooms and 39 preschool children on the playground. Children were not observed to be in distinguishable groups. The maximum group size for this age group is 20 children.

Required correction: Ensure group size is not exceeded at any time except as otherwise allowed in the rules and regulations. **Corrected at the time of visit. Children went inside to their respective classrooms and appropriate group sizes.** Respond with a plan for ongoing compliance by 12/15/25. Correct by 12/08/2025

On the preschool climber, observed a gap of approximately 0.5 inch between the top of the curved blue slide and the climbing platform, accessible to children and posing a potential safety hazard.

Observed several children playing on the swings and identified the resilient surfacing in the use zone was frozen. The surfacing directly under the swings measured six inches.

Required correction: Ensure all climbing equipment has resilient surfacing of at least six inches in the use zone surrounding the equipment. **Corrected at the time of visit; the children stopped playing on the swings.** No response required. Correct by 01/14/2025

Reviewed 13 staff files and identified A.'s file lacking documentation of an out-of-state clearance letter and documentation that the out-of-state requests have been submitted. A. has lived in CA and OH in the last 5 years.

Identified through Director K.'s statement that the Pre-K 112 class was on a field trip. Reviewed 3 health care plans and identified a child in the Pre-K 112 class requires emergency medication and observed the medication in the classroom. Identified through Director K.'s statement that staff did not carry the emergency medication with them on the field trip.

Required correction: Ensure that when away from the classroom, staff assigned to supervise the child carries the emergency medication. Correct immediately and respond with a plan for ongoing compliance by 1/31/24. Correct by 01/24/2024

Reviewed bus driver P.'s file and identified documentation of a Department-approved First Aid and CPR training to be missing. Multiple teachers were on the bus who had current CPR and First Aid certificates.

Required correction: Obtain current CPR and First Aid documentation for P. and ensure all drivers of vehicles owned or leased by the center have a current Department-approved First Aid and CPR training. Reviewed enrollment and permission forms and identified through documentation and Director K.'s statement that the center provides sunscreen for children and only specifies that sunscreen will be SPF 30 and does not specify the specific type of sunscreen provided.
