At the time of the visit, observed in all classrooms that the time a child arrives and leaves is not being recorded and staff are not taking attendance periodically throughout the day and staff are not taking attendance during transitions.

Required correction: Immediately ( 4/21/2025), the time a child arrives and leaves must be recorded. Staff must complete written attendance verification periodically throughout the day, including transitions. Submit a written statement of compliance by 5/21/2025. Correct by 04/21/2025

In Room 207, wipes labeled "keep out of reach of children" were on the counter, accessible to children. In Room 208, soap labeled "keep out of reach of children" was on the sink, accessible to children.

Required correction: Immediately, item labeled "keep out of reach of children" must be inaccessible to children. Corrected at the time of visit, the teachers placed the items on a high shelf, inaccessible to children. Submit a written statement of future compliance by 6/6/2024. Correct by 05/06/2024

In Room 218, licensed for children 2.5-3 years old, a small toy was in an unlocked cabinet accessible to children, causing a potential choking hazard.

In Room 219, 3 children were signed in, however, 6 children were in attendance. In Room 218, the teachers were unaware of how many children were in attendance. 16 were in attendance, however, only 10 were signd in.

Required correction: Immediately, the time a child arrives and leaves the center must be recorded and written attendance verification must be completed periodically througout the day. Corrected at the time of visit, the teachers and director corrected the attendance logs to reflect the correct amount of children in attendance. Submit a written statement of future compliance by 6/6/2024. Correct by 05/06/2024
