In classroom 137 there were 3 children missing the time of arrival on the sign in and out sheet.
Required correction: For security purposes, a sign-in/sign-out sheet or other mechanism for parents/guardians, or staff if children are being transported, must be maintained daily by the center. It must include, for each child in care, the date, the child's name, the time when the child arrived at and left the center, and the parent /guardian or staff member's signature or other unique identifier. For children who are transported, parent(s)/guardian(s) must verify the accuracy of the sign-in/sign-out sheet at least weekly. ***Corrected at the time of the visit the teacher marked the time of arrival for all 3 children on 3/11/2026.***Respond with a plan of ongoing compliance by 3/18/2026. During the walk through of the classrooms it was observed that classroom 145 had 20 children and is licensed for 19 children.
Required correction: The license room capacity must not be exceeded at any time including classroom 145. ***Corrected at the time of the visit staff members unlocked the cabinets making the area accessible to children. The unlocked cabinet area increased the over all room capacity to 20 children on 4/16/2025. Respond with a plan of on going compliance by 4/23/2025. Correct by 04/16/2025

Licensing specialist completed a walk through of the facility and all classrooms were outside on the playground and all 4 classes were lacking written attendance verification. Also in classroom 145 there were 10 children signed in and only 9 present. In classroom 137 the staff member stated they had 6 children and there were only 5 children signed in.
Required correction: Ensure that the times a child arrives and leaves the center must be recorded. Also staff members must complete written attendance verification through out the day including trabsistions. Correct immediately and respond with a plan of compliance by 5/31/2024. Correct by 05/24/2024

In classroom 136 there was a large bottle of Rocky Mountain Sunscreen labeled keep out of reach of children sitting on the children's cubbies and accessible to children.
Required correction: Ensure that all item labeled keep out of reach of children must be made inaccessible to children. ***Corrected at the time of the visit a staff member moved the bottle of sunscreen to a higher shelf making it inaccessible to children. No need for written statement of compliance.*** Correct by 09/13/2023

Outside on the preschool playground there was a large hole next to the cement ramp leading into the woodchip play area of the play ground. The hole is large and deep and posing a tripping hazard.
Required correction: Ensure that all outdoor play areas used for children must be checked daily and kept safe and free from hazard.
