Required correction: All individuals requiring a background check must review Navigating the Background Check Investigation Unit's (BIU) Webpage powerpoint. Submit verification that staff member has reviewed the powerpoint within 24 hours. Submit documentation of (Out of State Clearance Letters) for staff member with written response to the Specialist. In Early Preschool, the teacher to state she had 7 children and the attendance stated 7 children, but only had 6 children in the room. This is in direct violations of stipulation F on the probationary license. Stipulation F states "At all times, staff members shall know the attendance of the classroom to which they are assigned. Facility Director shall be able to account for all children in care at the facility." The 7th child had moved classrooms that morning.

Outside on the toddler playground, toddler 3 classroom was outside, the Licensing Specialist witnessed H. go inside leaving A. outside with 8 children ages 1.5 - 2 years of age for approximately 2 minutes. Previously cited on 1/14/25. This is a direct violation of stipulation C.

Required correction: Staff to child ratios must be maintained at all times. Corrected at the time of visit: A. took children inside the classroom. Submit a written statement of compliance to the Specialist. Correct by 04/28/2025

The Licensing Specialist witnessed, outside on the toddler playground, there were 8 toddlers playing while H. went inside the classroom and A. was in the door -way looking inside the classroom with her back to all the toddlers, for approximately 2 minutes. Previously cited on 5/23/22, 1/14/23, 1/25/24, 7/1/24, and 3/31/25. This is a direct violation of stipulation D

In toddler 1 classroom, no children were in attendance, 4 children were signed in. In toddler 2 classroom, 9 children were in attendance and 5 were signed in. In preschool 2 classroom 7 children were in attendance, 8 signed in. Previously cited on 10/2/24, 12/16/24, 3/31/25, and 4/25/25. This is a direct violation of stipulation F.
