There was two containers of diaper cream labeled "keep out of reach of children" accessible to children in a bin on a low self next to the restroom. There was a bottle of air freshener labeled "keep out of reach of children" accessible to children that was sitting on the back of the toilet accessible to children.

Required correction: Ensure that all hazardous items including diaper cream and air freshener must be inaccessible to children. ***Corrected at the time of the visit the provider places all items inacccesible to children on 3/17/2026. No response needed*** Correct by 03/17/2026

Outside on the porch area there was a lawn mower accessible to children. The lawn mower poses a possible safety hazard.

Required correction: Outdoor play space, including areas under decks must be free from safety hazards including, but not limited to, lawn mowers, tools, propane, gasoline, building scraps, and scrap metal.***Corrected at the time of the visit the provider's husband removed the lawn mower from the backyard on 3/17/2026.*** Respond with a plan for ongoing compliance by 4/17/2026. Reviewed the providers emergency drill log. The drill log was lacking lockdown and active shooter drills. Also the last Tornado drill was completed 10/23/2024.

Required correction: Emergency drills, lockdown and active shooter on premises drills must be held at least quarterly but often enough so that all occupants are familiar with the drill procedure and their conduct during a drill is a matter of established routine. The provider did not have a gate on the stairs leading to the upstairs of the home and had children 2 and under in care.

Required correction: Stairways of more than four (4) steps, in indoor and licensed outdoor areas, that are accessible to children must have gates that prevent access from the area being used when children under two (2) years old are present. ***Corrected at the time of the visit the provider places at gate at the bottom of the stairs preventing access to the children under 2 years old.
