reviewed 9 children files and observed Ch1, Ch2, to have expired medical statements as of 11.28.23 and 4.12.25. Observed ch8 and ch9 to also have expired medical health statements as of 10.9.25 and 7.29.25.

Required correction: Obtain current medical health statement for children and add to children files and send written response to specialist. Correct by 01/09/2026

Identified through staff statements from SM1, SM2, SM3 that staff will not offered an activity until after at least 40 min to children that have not fallen asleep they will sometimes move the ch to be near staff while they work on curriculum or observeation notes.

Required correction: Children who do not sleep after 30 min must be allowed to move to another area and provided with quiet toys and equipment to play with. Correct immediately and send written reponse to specialist. Correct by 09/23/2025

Tiger class SM1 stated that she was in a corner with ch1 patting them when she observed ch2 move towards door and she had no visual on the ch2 or ch3 near the door. Day of visit observed SM2 also sitting in same corner also having no direct visual of ch3 in hall near door. Observed in Lions room also a a ch4 placed behind staff rocking chair so SM 4 had no visual of Ch4 in the corner.

Required correction: All children must be under the direct supervision at all times of a qualified adult who has been assigned the responsibility to supervise. Correct immediately and send written response to specialist. Correct by 09/23/2025

Lions classroom adult scissors on low counter to be accessible to children and a knife pear knife about 3 inched blade on low counter next to trash can to be accessible to children causing a potential safety issue.

Required correction: Sharp tools and instruments must be stored in areas inaccessible to children correct immediately, corrected at time of visit Director locked scissors and took knife to kitchen area making items inaccessible to children. No response needed. Correct by 07/29/2025

In duckling room observed bumble bee art near door to have wiggle/googly eyes accessible to children. Duckling room is an infant room with children ages 6 weeks to 18 months.

Required correction: Toys, toy parts, furnishings, equipment and any materials accessible to cchildren under 3 yrs of ages must not be a choke hazard or able to inhaled, correct immediately, corrected at time of visit staff removed art work from classroom making them inaccesible to children. reviewed 19 staff files and observed D.H. to have lived out of the state of colorado over the last 5 yrs and to not have a 4 clearance letter. DOH 6/2022.

Required correction: All individuals requiring a background check must review Navigating the Background Check Investigation Unit's (BIU) Webpage powerpoint. Submit verification that staff members have reviewed the powerpoint within 24 hours. Submit documentation of Out of State background check, for staff members with written response to the Specialist. Correct by 01/23/2025
