Required correction: Items labeled 'keep out of reach of children' must be inaccessible to children. Correct immediately. Corrected on site. Director moved the soap and wipes to a shelf inaccessible to children. No response required. Correct by 01/14/2026

Thumb tacks being used to hang up art work above the hand washing sink used by children younger than three years of age.

Small broken crayons located on the coloring table in the outdoor play area pose a choking hazard and are accessible to children younger than three years of age.

Required correction: All individuals requiring a background check must review Navigating the Background Check Investigation Unit's (BIU) Webpage powerpoint. Submit verification that staff members have reviewed the powerpoint within 24 hours. Submit documentation of CBI clearance letters for staff members with written response to Specialist. Correct by 01/14/2026
