In the toddler 2 classroom, observed a container of crayons, some of the crayons in the container were broken and small enough to fit in a choke tube posing a potential choking hazard.
Required correction: Toys and equipment accessible to children under 3 years of age must not be a choke hazard. Corrected at time of visit- staff removed broken crayons from the container on 11/7/25. No response required. Correct by 11/07/2025

Reviewed 6 staff files and observed ECTs Q. (DOH: 8/20/18) and C.'s (2/18/99) files to be missing a current TRAILS clearance letter.
Required correction: All individuals requiring a background check must review 'Navigating the Background Check', the Investigation Unit's (BIU) Webpage powerpoint on the CDEC website. Submit verification (via email) that ECTs Q. and C. has reviewed the powerpoint within 24 hours. Correct immediately and submit documentation of TRAILS clearance letter for Q. and C. with written response to Specialist by 2/24/24. Correct by 01/24/2024

In Preschool 4 classroom, observed the following to be accessible to children, posing a potential safety/strangulation hazard: - A cord from the wall to the plant light to be loose and longer than 18 inches; and, - Fairy lights (approximately 6 feet) to be hanging loosely across the hooks underneath the classroom cubbies. In unsecured storage cabinet in School-Age hallway used to access the other classrooms, observed an orange extension cord (approximately 20 feet long).
Required correction: Keep all hazardous items inaccessible to children at all times. **Corrected at time of visit- Director stored items inaccessible to children on 1/24/24.** No response required. Correct by 01/24/2024
