Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Employees with the hire date of 2/5/24 and 1/2/24 did not have an Emergency Information form on file.

For children under three years of age, materials were not offered in sufficient quantity to allow all children to use them at some time during the day and to allow for a range of choices. In Space C104 and C106 there was not a sufficient quantity of materials in the center areas.

On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Employee with the hire date 07/29/24 did not have a TB test or screening before the first day of employment.

Parent's medication authorization did not include required information. In Space 8 and medication authorization form was not complete. Spaces that needed to be completed were completed by the teacher during our visit.

When medication was administered, documentation was not completed or maintained for 6 months and/or the documentation did not include the required information. In space C-106 teacher did not document the administer of medication given to the student on the medication sheet for 02/28/24.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In classroom C-104 there was an room spay aerosol can that was not in a locked cabinet.
